To request information about someone you think may have given birth while receiving services at Florence Crittenton, please see the guidelines listed below:

• All requests must be submitted in writing to us at 715 W. Mariposa St., Phoenix, AZ 85013
• Include the name of the birth mother along with the dates the birth mother may have been in our care
• Upon receipt of your request, you will be contacted by a member of our staff

Please Note:

• No identifying information can be given directly to the requestor
• Requestor will only be notified if we have the record
• In order to receive specific information on the record, the requestor must contact the Arizona Supreme Court Confidential Intermediary Program at 602.452.3378 or visit their website here.
